There are two Clinics at CDTC which provide primary care services: the Pediatric Primary Care Clinic and the Adult Clinic. Each of these provide medical care as part of the medical home model practiced in all of our programs and practitioners provide patients with the best quality of care in manner which respects our families cultural and linguistic diversity.
The Primary Care Clinics serve clients with medically complex conditions in Broward County and their families. Our physicians and their staff have many years of experience working with clients with special health care needs and many of the families who come to the clinic have developed long-lasting and trusting relationships with them, often from the birth of their children through their child’s transition into adult care.
CDTC’s Primary Care Clinics include a staff of physicians, nurse practitioners, nurses, medical assistants, referral specialists and a phlebotomist. With assistance from an “Access to Care” grant from the Community Foundation of Broward, CDTC was able to continue its “Pediatric Telephone Triage” service for families who call the Clinic with concerns that require immediate attention. This valuable service enables families to have their concerns addressed via telephone by medical professionals familiar with their children’s diagnoses or, when necessary, arrange for a same day appointment. For many of the families served by the clinic, lack of transportation or the cost of transportation is often a barrier to their access to care, so the Telephone Triage line makes it easier for them to receive guidance from the medical team without having to make a trip to the Clinic or the ER.
This past year, as part of the federal quality measures across the health care industry, CDTC implemented a new Electronic Health Records system and has been charting, prescribing and placing orders electronically. In addition, the Center began new Patient Centered Medical Home (PCMH) and Meaningful Use (MU) quality initiatives and has been working with the Clinic staff and Broward Health IT to start tracking key data indicators in the Clinic throughout the coming year.
