The Pediatric Primary Care Clinic provides medical care as part of the medical home model practiced in all our programs and practitioners provide patients with the best quality of care in manner which respects our families cultural and linguistic diversity.

The Primary Care Clinics serve clients with medically complex conditions in Broward County and their families. Our physicians and their staff have many years of experience working with clients with special health care needs and many of the families who come to the clinic have developed long-lasting and trusting relationships with them, often from the birth of their children through their child’s transition into adult care. 

Unique to our service delivery is the purposeful integration of medical and oral health care.  Co-located within CDTC are the pediatric and dental clinics, allowing for the most comprehensive approach to health care for our clients.  Our Pediatric Primary Care Clinic, Dental Clinic and Medical Home programs that offer medical case management work together to give each of our children and families a dedicated Care Team they can always count on.

CDTC is an NCQA recognized Patient-Centered Medical Home, using a model of care that puts patients at the forefront. PCMH’s build better relationships between patients and their clinical care teams, improving quality of care and the patient experience, while reducing health care costs.
